We will ask that you complete a New Patient form in addition to signing our Notice of Privacy Practices form on your first visit.  All of these forms can be found by selecting Patient Forms.  Please feel free to complete these forms prior to your visit.  However, all forms must be signed in the presence of one of our staff members.

During your first visit, we will perform a complete diagnostic examination and evaluation.  We will take an ultrasound of your jaw joints, impressions of your teeth for study models, special diagnostic X-rays and a computer analysis of your jaw.  Following this evaluation, Dr. Auvenshine will sit down with you and discuss his findings and treatment recommendations.
